The 28-year-old openside flanker has suffered his third ankle injury in just seven months, putting his availability for the tournament in serious doubt. This latest setback could be a massive blow for England, given Underhill's impressive track record. He played every game in the 2024 Six Nations and featured in England's summer showdowns against Japan and the All Blacks.
Future up in the air
Despite his contributions, Underhill missed out on a central contract, casting uncertainty over his long-term future in the Premiership. With French clubs circling, the Bath star may soon be tempted to take his talents overseas.
Bath boss raises concerns
Bath head of rugby Johann van Graan confirmed that Underhill will sit out their Investec Champions Cup clash against Clermont Auvergne this weekend, further fuelling doubts about his Six Nations involvement.
"Sam took a knock to his ankle setting up Max Ojomoh's try," Van Graan explained. "He played on, but it was quite sore after the game, and that's a bit of a worry."
A big-game player
Van Graan was quick to highlight Underhill's importance, saying: "He's a big-game player. Look back at the Premiership final—he was one of our best that day."

While Bath waits for further medical assessments, the prospect of Underhill missing the Six Nations looms large. For England fans, it's a tense wait to see if one of their key men will be ready to don the white jersey come February.
